CU Succeed Enrollment Form.
If your teen is not yet a member of Arizona Central Credit Union:
- Complete our Membership Application (requires Adobe Acrobat Reader®).
- Mark the CU Succeed box on the Membership Application to indicate you would like to enroll your child.
- Include an initial deposit of at least $25 with your membership application to open your teen's account. Your child should be listed as the primary owner on the account.
